Three questionnaires, published in full. You can read them before you decide to run anything, use them as a reference for a process of your own, or hand them to a board that wants to know what it is agreeing to.
The wording is fixed. Once a version is published we do not edit the text of a question, because changing it would quietly break every comparison to a previous year and to other organizations. A change means a new version, recorded as such.
